What is the Respiratory Service?

The Community Respiratory Team (nursing and physiotherapy ) was established to provide care and support to patients in their own home who have a diagnosis of COPD or other respiratory diseases.

The aim of the service is to enable Primary Care Professionals to identify and manage respiratory patients with complex needs in the primary care setting.

This involves 4 different aspects of respiratory care:

  1. Respiratory Enhanced Discharge Scheme,
  2. Chronic Disease Management,
  3. Pulmonary Rehabilitation and
  4. Respiratory Support Group.

Community management includes:

  • Spirometry
  • Medication management including Inhaler
  • technique/compliance and nebuliser therapy management
  • Long term use of oxygen therapy education and management
  • Support visit – Patient starting an exacerbation
  • Airway clearance
  • Breathlessness management
  • Pacing
  • Ambulatory oxygen assessment and provision
  • Palliative care
  • Promoting expert self-management plan
  • Smoking cessation advice
  • Home exercise programmes
  • Pulmonary rehabilitation
  • Telehealth
