The Community Outreach Team

The Community Outreach Team offers support to people with current, severe and enduring mental health difficulties, who find it difficult to cope and live with the problems they experience. In particular, the team aim to develop contact with people who, for whatever reason, are reluctant to engage with mental health services.

The Team works over seven days per week, including public holidays, Monday to Friday 9am-8pm and from 9am-5pm on Saturday and Sunday.

The team aims to work in partnership with clients in identifying individual needs so that the most appropriate package of care can be provided. Central to the aims of the service are to help clients avoid admission to hospital and enjoy a better quality of life. Another key aim is to develop partnerships with clients and their carers in order that the optimum level of support is provided.

Each client is allocated a member of the team who is responsible for co-ordinating the provision of all mental health care and treatment. On occasions, due to individual needs, it may be necessary for other members of staff to provide such care.

Clients are encouraged to become actively involved and to participate in planning their care and reviewing progress. Also, it is recognised that the involvement of carers is often important and beneficial and therefore, with the consent of the client, the team will involve carers to promote the well-being of all concerned.

Depending on individual need, the team may involve other professionals and organisations in providing the care and treatment needed.
