The Community Mental Health TeamThis Team offers a service for adults within the age range of 18 – 64 living in the North Down and Ards Area, who have current primary mental health needs. The team offers time–limited and long-term interventions depending on assessment of needs. The team comprises of:

Community Mental Health Team 3 Church Street Newtownards Tel: 028 9151 2155 In an emergency, the out of hours GP on-call can be contacted by calling: Telephone 02891822344 Aim of Care The team aims to respect service users views and that of the carers. The team aims to complete its work, promoting self help and recovery before referring clients back to their GP within an appropriate time scale. How does the Community Mental Health Team Work? All clients will have a key worker who in partnership, will complete a care plan which is tailored to individual need. Treatments are aimed at dealing with difficulties with mood, behaviour, medication management or relationships. All clients are encouraged to become involved in their own care package and will be supported in applying self-help methods. All clients need to give consent before staff can provide any care or treatment. A copy of the care plan will be given to the client and the professional who makes the referral to this service. It is our intention to respect people’s wishes and to keep information confidential. In the absence of your key worker it may be necessary for another team member to provide support to you. |
