Diabetes Service
AIMS
The aims of the Local District Diabetes Network are:
- To minimise premature morbidity and mortality in those with diabetes
- To maximise quality of life by detecting and treating the disease and its complications at an early stage
- To provide equal access to locally accessible high quality diabetes care for all the residents of the area.
- To work towards the establishment of a district wide diabetes register through audit and clinical governance to ensure the delivery of high quality diabetes care
OBJECTIVES
- To implement a planned programme of care for all patients with diabetes undertaken in an integrated manner involving all relevant specialists.
- To ensure that all Type I diabetic patients are seen (at least annually) at the hospital diabetes clinic.
- To offer all patients with Type 2 diabetes a high standard of care, including annual review in primary or secondary care clinics
- To ensure that inpatient diabetes care is of a high standard with timely referrals.
- To ensure referral of any patient with diabetes who shows signs of potential complications or instability in blood glucose concentrations or other cardiovascular risk factors, for specialist assessment.
- To agree an individual management plan with the patient.
- To audit the care of the patients with diabetes by following a well-defined and agreed protocol.
- To identify and register all those patients diagnosed with diabetes in the area.
- To keep the register updated and initialise a systematic call and recall system at the hospital or in general practice.
The Diabetes Team includes many different health care professionals all working together to bring you the best possible diabetes service. The team is comprised of doctors, specialist nurses, a specialist midwife, dietitians, podiatrists, psychologists, retinal screeners, secretaries, clerical officers and a diabetes facilitator.
